The online MDA store features a variety of items including the popular Lab Forms. You also can make a donation to MODentPAC, MDA Foundation or the Dental Well Being Foundation. If you do not wish to use the online store you may download and complete a printable order form.
- All items are shipped within 4-6 business days by U.S. Postal Service mail. Discount pricing available for members only; your ADA number will be verified.
- The MDA is no longer printing Stop the Pop or Start Right brochures, but you can download the brochure on our Oral Health Education Resources page.
