• There are two ways to register for Connect4Success: Online registration or download the  fillable PDF form, complete and return to the MDA. Advance registration closes (online or PDF) Sunday, June 16.
  • The online registration will allow you to add multiple registrants, guests and children, and pay at the end.
  • If completing the PDF form, there should be one registration per person, except Guest/Child information. In this case, you may register your guest/child(ren) on the same form if they are participating in free events and/or purchasing a Guest Meal Only Option.
  • If your guest is attending CE courses, they must complete a separate registration form. There is no registration fee for guests/child(ren), but there is a fee for the Guest Meal Only Option (see below, under Special Events & Meals). 
• Fees and Cancellations: A $50 cancellation fee per registration will be charged through June 16, 2024; no refunds after this date (Request for a refund/cancellation must be emailed to Mandy Lewis). All onsite registrations at Margaritaville Lake Resort are an additional $25 fee.
• Some CE has limited availability! Please note this when registering!
  • Endodontic Instrumentation and Obturation (Friday afternoon) is a prerequisite for attending the Saturday morning hands-on workshop, Irrigation and Hands-On Workshop. The max attendance in the hands-on workshop is 35 attendees. There is an additional $75 Fee for the hands-on course.
  • Vendor CE options (Breakfast Club and Lunch Bunch) are free, but have limited capacity; you must register to attend.

• Registration fees for dentists and dental team member include all courses, meals and most events.
• The Salt & Lime Reception (Thu) is free to all registrants (guests and children), but we do ask you to indicate if you/guest/children will be attending or not to assist the MDA in planning.
• The New Dentist Hangout (Thu) is free to adult registrants and a guest BUT the primary (dentist) registrant must have graduated from dental school 10 years ago or fewer.
• A Guest Meal Only Option is available to purchase for adults and children who do not register to attend CE but do attend the meeting as a guest. This meal option includes 2 Receptions (plus drink tickets), 2 Breakfasts, 1Lunch and All Breaks. Adults $65, Children (ages 9-18) $30, 8 and under FREE.

Salt & Lime Welcome Reception & Exhibits Preview
For guests arriving Thursday evening, you're invited to join us from 5:30p-7:30p in the Exhibit Hall for a welcome reception and exhibits preview. Enjoy drinks, snacks and sweet treats (for the kids and kids at heart—families welcome). Attendees can meet and greet one another while spending time with exhibitors before CE begins the next day.

New Dentist Hangout 
The MDA New Dentist Committee invites all new dentist attendees to drop by and hang out together Thursday, June 20 at 7:30p (immediately after Salt & Lime Reception). We’ll have pizza and beverages and play a few games for prizes. This event is at Margaritaville’s Frank and Lola’s where there’s shuffleboard, pool tables and other games. Sponsored by Compass Health Network

ReConnect Reception 
End a great day of CE at the ReConnect Reception! Once again this year it's outside on JB’s Deck, and it’s Tropical Style, so don’t forget your Hawaiian shirt and flip flops! From 4:30p-6p, attendees can relax while enjoying beverages and appetizers among the camaraderie of peers and exhibitors. And we’ll announce the final Connect4Cash Foundation Raffle winners at the Reception! Sponsored by MDIS

The MDA moved its annual House of Delegates meeting to be in conjunction with Connect4Success. Below is a preliminary schedule for the June 2024 House of Delegates. We’ve pushed the House of Delegates meeting back until 2pm to allow those attending C4S a small break and to allow those driving in to have more time to arrive. As always, the times for day one depend on the time of adjournment of the First Session of the House and how long the Reference Committee Hearings last. Reference Committee Executive Session will begin following the adjournment of the hearings.

Saturday, June 22, 2024 (First caucuses held prior to C4S)
12pm: C4S ends (Lunch on your own)
2-4pm: 1st House Session and Reference Committee Hearings
4-5:30pm: Reference Committee Executive Sessions
6:30-7:30pm: Reception
7-9pm: Awards Dinner

Sunday, June 23, 2024
8:30-10am: 2nd Caucus Breakfasts
10am-Noon: 2nd House Session
Close of House
No Board of Trustees Meeting after
